Efectividad de barniz de flúor vs sellantes: revisión sistemática

Resumen
Antecedentes: los términos de promoción y prevención en salud bucal aparecen con poca fuerza a mediados del siglo XX. En los años sesenta, para proteger los dientes de la caries, hacen su aparición soluciones de fluoruro de sodio y sellantes de fosas y fisuras. Los dos materiales se basan en la premisa de que la prevención en salud bucal debe estar orientada a combatir los factores de riesgo incluyendo el proporcionar al huésped una mayor resistencia. Estas estrategias han evolucionado en cuanto a composición, presentación y formas de aplicación. Son utilizadas simultáneamente en edad temprana. Objetivo: realizar una revisión sistemática de la literatura en las bases Scopus, Pubmed, Cochrane y Embase sobre la efectividad de la aplicación de sellantes vs. barniz fluorado en niños de 2 a 12 años. Métodos: se realizó una revisión sistemática con ensayos clínicos, se seleccionaron los estudios que cumplieran los criterios de inclusión (texto completo a partir de 2007 participantes de ambos sexos con edades entre los 2 y 12 años en inglés, español y portugués). Las variables analizadas fueron nombre del primer autor, base de datos, año de publicación, idioma, país de publicación, efectividad, edad y sexo de los participantes, casa fabricante y nombre comercial del producto, y calificación según la Declaración Consort, entre otros. Se calcularon frecuencias y proporciones para las variables cualitativas y medidas de tendencia central y dispersión para las cuantitativas. Resultados: Se evaluaron 81 artículos de los cuales se seleccionaron ocho que cumplían con los criterios de inclusión (Figura 2). Se encontró que el número mayor de personas evaluadas la hicieron de Neidel M y colaboradores con 1.015 participantes. El el estudio con el menor tiempo de seguimiento fue el de Kalnina J, Honkala S y colaboradores con un total de doce meses cada uno. Uno de los artículos que presentó un alto número de autores, fue el de chestnutt I.G y colaboradores con 25 autores siendo el primero de Inglaterra (Tabla 2). De igual manera este autor se repitió en dos de los ocho artículos. Si bien la búsqueda se realizó a partir de 2007, únicamente se encontraron artículos desde 2012 a 2017. Durante los años 2012, 2013, 2016 y 2017 se publicaron dos artículos por año relacionados con el tema del trabajo (Tabla 2). Las revistas con mayor número de publicaciones sobre la efectividad del barniz fluorado vs. La aplicación de sellantes fueron Caries Research y Journal of Dental Research con 2 (25 %) artículos, cada una (Tabla 3). La casa fabricante del sellante mayormente usado fue 3M en 4 (50,0 %) estudios (Tabla 4) y de barniz de flúor fue Colgate en 4 (50 %) trabajos (Tabla 4). Al revisar la efectividad de la intervención, los trabajos evaluados concluyeron que el sellante fue más efectivo en 3 (37,5%). Sin embargo, 4 (50%) estudios reportaron hallazgos similares entre la aplicación del barniz fluorado y del sellante. Todos los autores recomiendan mayor investigación al respecto (Tabla 5). Conclusiones: En tres artículos se determinó que fue más efectiva la aplicación de sellantes vs. El barniz fluorado. No obstante, los resultados fueron similares en cuatro artículos. En total, se encontraron ocho ensayos clínicos que evaluaron la aplicación de sellantes vs. La aplicación de barniz de flúor en niños de 2 a 12 años. Fue más recomendada la aplicación de sellantes aun cuando los resultados obtenidos en los estudios clínicos fue similar o con diferencias no significativas.
Resumen
Background: the terms of promotion and prevention in oral health appear with little force in the mid-twentieth century. In the sixties, to protect the teeth from decay, solutions of sodium fluoride and sealants from pits and fissures appear. The two materials are based on the premise that prevention in oral health should be aimed at combating risk factors including providing the host with greater resistance. These strategies have evolved in terms of composition, presentation and application forms. They are used simultaneously at an early age. Objective: to carry out a systematic review of the literature in the Scopus, Pubmed, Cochrane and Embase databases on the effectiveness of the application of sealants vs. fluoridated varnish in children from 2 to 12 years. Methods: a systematic review was carried out with clinical trials, and studies that met the inclusion criteria were selected (full text from 2007 onwards of both sexes with ages between 2 and 12 in English, Spanish and Portuguese). The analyzed variables were name of the first author, database, year of publication, language, country of publication, effectiveness, age and sex of the participants, manufacturer's house and commercial name of the product, and qualification according to the Consort Declaration, among others. Frequencies and proportions were calculated for the qualitative variables and measures of central tendency and dispersion for the quantitative ones. Results: 81 articles were evaluated, of which eight were selected that met the inclusion criteria (Figure 2). It was found that the largest number of people evaluated was made by Neidel M and collaborators with 1,015 participants. The study with the shortest follow-up time was that of Kalnina J, Honkala S and collaborators with a total of twelve months each. One of the articles that presented a high number of authors was that of chestnutt I.G and collaborators with 25 authors being the first of England (Table 2). During the years 2012, 2013, 2016 and 2017 two articles per year related to the subject of work were published (Table 2). The journals with the highest number of publications on the effectiveness of fluoride varnish vs. The application of sealants were Caries Research and Journal of Dental Research with 2 (25%) articles, each (Table 3). The house manufacturer of the sealant mostly used was 3M in 4 (50.0%) studies (Table 4) and fluoride varnish was Colgate in 4 (50%) works (Table 4). When reviewing the effectiveness of the intervention, the evaluated works concluded that the sealant was more effective in 3 (37.5%). However, 4 (50%) studies reported similar findings between the application of fluoride varnish and sealant. All authors recommend further research in this regard (Table 5). Conclusions: In three articles it was determined that the application of sealants was more effective. The fluorinated varnish. However, the results were similar in four articles.
