dc.descriptionLa rehabilitación oral de pacientes totalmente edéntulos mediante prótesis híbridas implantosoportadas presentó desafíos biomecánicos asociados con la distribución de cargas, la estabilidad oclusal y el comportamiento estructural de la subestructura protésica, lo que motivó la evaluación integrada de las condiciones de diseño (materiales y geometrías) y técnicas quirúrgicas. Para comprender la influencia de estas variables sobre la interfaz hueso–implante y sobre la respuesta mecánica de barras implantosoportadas en el maxilar inferior, se desarrollaron análisis computacionales in silico mediante el método de elementos finitos, que permitieron comparar múltiples combinaciones bajo un esquema oclusal funcional estandarizado. Los resultados numéricos fueron posteriormente sometidos a validación experta, con el fin de garantizar coherencia clínica y biomecánica respecto a los patrones observados en rehabilitaciones mandibulares con cuatro implantes rectos. Adicionalmente, se realizaron ensayos in vitro dirigidos a caracterizar el desempeño de las subestructuras seleccionadas, lo que permitió contrastar los hallazgos experimentales con las predicciones del modelo computacional. Los análisis integrados evidenciaron variaciones significativas en los esfuerzos transmitidos al hueso cortical y trabecular, así como en las deformaciones de las barras, según el material, la geometría y la técnica quirúrgica empleada, identificándose configuraciones con comportamiento biomecánico superior. La síntesis de estos hallazgos proporcionó fundamentos científicos y técnicos para la formulación de un protocolo orientado a optimizar la predictibilidad, estabilidad y durabilidad de las rehabilitaciones orales implantosoportadas en pacientes totalmente edéntulos del maxilar inferior.
dc.description.abstractThe oral rehabilitation of fully edentulous patients using implant-supported hybrid prostheses presented biomechanical challenges associated with load distribution, occlusal stability, and the structural behavior of the prosthetic substructure, which motivated the integrated evaluation of the materials, geometries, and surgical techniques used for their design. To understand the influence of these variables on the bone–implant interface and on the mechanical response of implantsupported bars in the mandible, in silico computational analyses were conducted using the finite element method, allowing the comparison of multiple combinations under a standardized functional occlusal scheme. The numerical results were subsequently subjected to expert validation to ensure clinical and biomechanical coherence with the patterns observed in mandibular rehabilitations using four straight implants. Additionally, in vitro tests were carried out to characterize the performance of the selected substructures, enabling comparison between the experimental findings and the predictions of the computational model. The integrated analyses revealed significant variations in the stresses transmitted to cortical and trabecular bone, as well as in bar deformations, depending on the material, geometry, and surgical technique employed, identifying configurations with superior biomechanical behaviour. The synthesis of these findings provided scientific and technical foundations for the formulation of a protocol aimed at optimizing the predictability, stability, and durability of implant-supported oral rehabilitations in fully edentulous patients of the mandible.
