Pronóstico de la endodoncia convencional después de negociar los conductos con instrumento separado. Revisión sistemática

dc.descriptionIntroducción: En la práctica clínica de la endodoncia se puede producir la separación de instrumentos intraconductos, en cualquier fase del procedimiento, instrumentos tales como Gates Glidden, limas de Níquel Titanio, limas de acero inoxidable, puntas ultrasónicas entre otros. Se pueden separar por fatiga cíclica o torsional o por combinación de ambas; cuando se producen fuerzas repetidas de compresión y tracción que actúan sobre la parte externa de la lima que gira dentro de un conducto curvo, o cuando la punta de la lima se queda trabada, pero el vástago de la lima sigue en movimiento por el motor. Metodología: El estudio es una revisión sistemática. Las bases de datos que se establecieron para la búsqueda fueron: Pubmed, Cochrane, Scopus, Lilacs, Embase. Por medio de esta búsqueda, se obtuvo 501 artículos, publicados entre 1999 y 2019 de los cuales, se obtuvo 484 por duplicidad de algunos de ellos. Se seleccionó un total de 17 para realizar lectura del resumen y después de aplicar los criterios de selección, se obtuvo 7 estudios y el análisis de todas las variables se realizó sobre la muestra reportada de 401 dientes. Para el análisis del riesgo de sesgo se elaboró una herramienta con base en la guía Prisma y las directrices CARE y Faggion. Resultados: De las siete publicaciones, 4 fueron series de caso, 1 estudio in vitro, 1 descriptivo de corte transversal y 1 reporte de caso. Los métodos de evaluación de la efectividad más frecuentes fueron los radiográficos en 5 publicaciones. Los instrumentos separados, más comúnmente reportados fueron las limas rotatorias en 6 estudios y el tiempo de seguimiento estaba entre 3 y 5 años. Conclusión: En los siete artículos evaluados, la separación intraconductos de los instrumentos presenta riesgo entre bajo y medio y el pronóstico de los dientes se encuentra entre buena y regular.spa
dc.description.abstractIntroduction: In the clinical practice of endodontics, separation of intracanal instruments can occur, at any stage of the procedure, instruments such as Gates Glidden burs, Nickel Titanium files, stainless steel files, ultrasonic tips, among others. They can be separated by cyclic or torsional fatigue or by a combination of both; when there are repeated compressive and tensile forces acting on the external part of the file rotating inside a curved canal, or when the tip of the file gets stuck, but the file shaft is still moved by the motor. Methodology: The study is a systematic review. The databases established for the search were: Pubmed, Cochrane, Scopus, Lilacs, Embase. Through this search, 501 articles were obtained, published between 1999 and 2019, of which 484 were obtained due to duplication of some of them. A total of 17 were selected to read the summary and after applying the selection criteria, 7 studies were obtained and the analysis of all variables was performed on the reported sample of 401 teeth. For the analysis of the risk of bias, a tool was developed based on the Prisma guide and the CARE and Faggion guidelines. Results: Of the seven publications, 4 were case series, 1 in vitro study, 1 descriptive cross-sectional study and 1 case report. The most frequent effectiveness evaluation methods were radiographic in 5 publications. The most commonly reported separate instruments were rotary files in 6 studies and the follow-up time was between 3 and 5 years. Conclusion: In the seven articles evaluated, the intracanal separation of the instruments presents a risk between low and medium and the prognosis of the teeth is between good and regular.spa
