Endodontic microsurgery is performed with the aim of inducing a repair of periapical tissue altered by infectious processes that are associated with previous endodontic failures, trauma, complex radicular anatomy and undesirable events during the instrumentation process. This article shows the evolution of 12 cases of teeth that were treated with endodontic microsurgery, using Endosequence® bioceramics and Biodentine® as a filling material. The analysis was performed by CBCT cone beam tomography, which allowed to evaluate bone changes after periradicular surgery. A descriptive observational study of 12 cases was performed in 7 controlled patients at 3, 5, 12, 18 and 24 months, measuring the width, height, depth and perimeter of the lesion. This evaluation allowed to determine the bone healing process. Three cases presented complete scarring at 18 months and 3 months respectively, 8 cases showed volumetric changes from 3 months after the surgical procedure that with the passage of the evaluation time were reflected in reduction of the size of the lesion and 1 case considered as a failure 3 months after surgery; These results show favorable changes in bone healing in most cases. Conclusions: It was observed favorable bone changes in the different periods of time to be evaluated, using CBCT independent of the size of the lesion.spa
