Técnica Biomimética como tratamiento alternativo para la conservación del diente tratado endodónticamente y estructuralmente comprometido. Caso clínico.

| dc.description | Problema: La restauración de dientes endodónticamente tratados presenta un desafío significativo debido a las deficiencias estructurales intrínsecas, la pérdida de tejido y la presencia de restauraciones previas Método: la introducción de materiales y procedimientos adhesivos ha transformado la forma de restaurar los dientes tratados endodónticamente y con compromiso estructural. Las fibras de polietileno tienen un módulo de elasticidad similar a la dentina que se puede utilizar como material para la elaboración de muñones, por ello han ido reemplazando el uso de retenedores intrarradiculares y han ganado popularidad, ya que con el uso de estos materiales se preserva más estructura dental, se distribuyen las fuerzas de manera uniforme a lo largo del eje longitudinal del diente y, además, se adaptan a las paredes del conducto radicular, mejorando la resistencia. Este enfoque conservador no solo evita la necesidad de materiales protésicos como retenedores intraradiculares para sostener una corona, que, aunque efectivos, pueden no ofrecer una integración óptima con los tejidos naturales del diente, además, también permite una restauración que no solo refuerce el diente, sino también que sea funcional y estética. Objetivo: este estudio es presentar un caso clínico, con un enfoque biomimético como tratamiento alternativo para la conservación de dientes tratados endodónticamente que presentan compromiso estructural significativo, con un seguimiento a seis meses. Resultados: el caso clínico demuestra a la fecha que la técnica biomimética ha sido efectiva para rehabilitar los dientes con compromiso estructural. La ausencia de lesión periapical, la disminución del ensanchamiento del ligamento periodontal, la recuperación de los tejidos blandos, pero sobre todo mantener aun en boca lo que ya se daba por perdido, son indicadores del éxito del tratamiento. Conclusiones: los materiales y la aplicación de técnicas biomiméticas está permitiendo restaurar dientes en forma menos invasiva para reconstrucción directa de dientes con gran pérdida estructural de tejido dental, usar fibras de polietileno en conjunto con resinas para reforzar la raíz y estructura coronal parece ser una opción de tratamiento, lo que demuestra ser una alternativa viable para mantener la funcionalidad del diente | |
| dc.description.abstract | Problem: the restoration of endodontically treated teeth presents a significant challenge due to intrinsic structural deficiencies, tissue loss, and the presence of previous restorations. Method: the introduction of adhesive materials and procedures has transformed how endodontically treated and structurally compromised teeth are restored. Polyethylene fibers, with an elastic modulus similar to dentin, can be used as a material for core buildups. For this reason, they have been replacing the use of intraradicular retainers and have gained popularity. The use of these materials preserves more tooth structure, distributes forces uniformly along the tooth's longitudinal axis, and adapts to the root canal walls, improving resistance. This conservative approach not only avoids the need for prosthetic materials like intraradicular retainers to support a crown, which, while effective, may not offer optimal integration with the natural tooth tissues, but also allows for a restoration that not only reinforces the tooth but is also functional and aesthetic. Objective: This study aims to present a clinical case with a biomimetic approach as an alternative treatment for the conservation of endodontically treated teeth that present significant structural compromise, with a six-month follow-up. Results: The clinical case to date demonstrates that the biomimetic technique has been effective in rehabilitating structurally compromised teeth. The absence of periapical lesions, the reduction in periodontal ligament widening, the recovery of soft tissues, and, above all, keeping what was previously considered lost still in the mouth, are indicators of treatment success. Conclusions: Materials and the application of biomimetic techniques are allowing for less invasive restoration of teeth for direct reconstruction of teeth with significant structural loss of dental tissue. The use of polyethylene fibers in conjunction with resins to reinforce the root and coronal structure appears to be a viable treatment option, demonstrating to be an effective alternative for maintaining tooth functionality. | |
