Uso de lente terapéutico y membrana amniótica y su efectividad en trastornos de superficie ocular: revisión de literatura

dc.descriptionObjetivo: Determinar la efectividad del uso combinado de lente terapéutico y membrana amniótica en trastornos de superficie ocular, según lo reportado en la literatura en los últimos 10 años. Metodología: Se realizo un estudio secundario tipo revisión sistemática de la literatura, en la cual se incluyó investigaciones con información relacionada a trastornos de superficie ocular y su manejo con membrana amniótica y lente de contacto terapéutico. Para la consecución de la información se construye diferentes ecuaciones de búsqueda para indagar en las bases de datos como: pubmed, scopus, Taylor & Francis, Nature, clinicalKey, Science Direct, Sage Journals, ovid, DialNet, Lilacs, SpringerLink, ProQuest, Redalic, Scielo, embase, Jama Network y otros de búsqueda libre. De los artículos se extrajo información referente a los datos bibliométricos de los mismos (autor, año, país, idioma y tipo de estudio) y clínicos con relación al tiempo de cicatrización de la córnea, desenlace a nivel visual, patología que se trató y el tipo de intervención realizada. Resultados: Se extrajeron 10 artículos que cumplieron los criterios de inclusión; de los cuales 6 estaban en inglés, 2 en chino, y 2 en coreano. El 80% correspondía a estudios prospectivo y 20% a retrospectivos. Los resultados fueron bastante heterogéneos desde la variabilidad en el tiempo de recuperación del tejido que oscila desde 3 días a 30 días según reporta 7 estudios y otros 3 que afirman también mejoría de la condición presentada. También se reporta una notable tasa de recuperación completa y la mejoría de la agudeza visual luego del tratamiento. Dentro de las patologías que se trataron se menciona defecto epitelial persistente, queratitis punteada, lesión química aguda en córnea, enfermedad de ojo seco, úlcera corneal, cicatrización corneal post PRK y otras afecciones de superficie. Conclusiones: Se presentan otras opciones terapéuticas para manejar diversas alteraciones de la superficie ocular. Sin embargo, se obtienen mejores resultados en lesiones superficiales y síntomas cuando se apoya la membrana amniótica con un lente terapéutico.
dc.description.abstractObjective: To determine the effectiveness of the combined use of therapeutic lens and amniotic membrane in ocular surface disorders, as reported in the literature in the last 10 years. Methodology: A secondary study was conducted in the form of a systematic review of the literature, which included research with information related to ocular surface disorders and their management with amniotic membrane and therapeutic contact lens. To obtain the information, different search equations are built to search databases such as: PubMed, scopus, Taylor & Francis, Nature, ClinicalKey, Science Direct, Sage Journals, Ovid, DialNet, Lilacs, SpringerLink, ProQuest, Redalic, Scielo, embase, Jama Network and other free search databases. From the articles, information was extracted regarding the bibliometric data of the articles (author, year, country, language, and type of study) and clinical data in relation to the healing time of the cornea, visual outcome, pathology treated, and the type of intervention performed. Results: ten articles that met the inclusion criteria were extracted; of which six were in English, two in Chinese, and two in Korean. Eighty percent corresponded to prospective studies and 20% to retrospective studies. The results were quite heterogeneous, from the variability in tissue recovery time, ranging from 3 days to 30 days, as reported by seven studies and another three that also affirm improvement of the condition presented. A remarkable rate of complete recovery and improvement in visual acuity after treatment are also reported. Among the pathologies that were treated are persistent epithelial defects, punctate keratitis, acute chemical injury to the cornea, dry eye disease, corneal ulcer, post PRK corneal scarring and other surface conditions. Conclusions: Other therapeutic options are presented to manage various ocular surface alterations. However, better results are obtained in superficial lesions and symptoms when the amniotic membrane is supported with a therapeutic lens.
