Agudización post-endodoncia relacionada con la necrosis pulpar y la periodontitis apical asintomática reportada en la literatura: Scoping Review

dc.descriptionAntecedentes: La agudización post-endodoncia o Flare-Up se refiere al desarrollo de dolor y/o edema que comienza unas pocas horas o días después de los tratamientos endodónticos. Este dolor es de gravedad significativa por lo que requiere una visita no programada para un tratamiento de urgencia. Las tasas reportadas en la literatura sobre agudizaciones van del 1,4% al 16%. Esta investigación busca a través de un Scoping Review actualizar, reunir y sintetizar la magnitud cuantitativa de la relación reportada en la literatura, entre la agudización post-endodoncia con la necrosis pulpar y la periodontitis apical asintomática. Materiales y métodos: Se realizo una búsqueda de artículos de los últimos 10 años en las bases de datos Pubmed, ScienceDirect, Ebsco, Lilacs, Cochrane y Google Scholar según los criterios de inclusión y exclusión definidos previamente. La evaluación de la calidad metodológica, riesgo de sesgo y la extracción de datos se realizaron de forma independiente y por duplicado. Los principales resultados se extrajeron en una sábana de resultados diseñada por los autores. Resultados: Se seleccionaron 4 estudios de los cuales, 3 fueron de casos y controles y 1 de cohorte. Se encontró un riesgo bajo de sesgo y una calidad metodológica media según las guías empleadas para su evaluación. Ninguno de los estudios incluidos mostró la magnitud de la relación entre la necrosis pulpar, la periodontitis apical y el Flare Up. Conclusiones: La magnitud de la relación entre la necrosis pulpar y la periodontitis apical asintomática con el Flare Up no pudo ser establecida debido a la falta de información y datos reportados por los artículos incluidos en el presente trabajo.spa
dc.description.abstractBackground: Post-endodontic flare-up or flare-up refers to the development of pain and/or edema that begins a few hours or days after endodontic treatments. This pain is severe enough to require an unscheduled visit for emergency treatment. The rates reported in the literature on exacerbations range from 1.4% to 16%. Through a Scoping Review, this research seeks to update, gather and synthesize the quantitative magnitude of the relationship reported in the literature between post-endodontic exacerbation with pulpal necrosis and asymptomatic apical periodontitis. Materials and methods: A search for articles from the last 10 years was carried out in the Pubmed, ScienceDirect, Ebsco, Lilacs, Cochrane and Google Scholar databases according to the previously defined inclusion and exclusion criteria. Assessment of methodological quality, risk of bias, and data extraction were performed independently and in duplicate. The main results were extracted into a results sheet designed by the authors. Results: 4 studies were selected, 3 of which were case-control and 1 cohort. A low risk of bias and a medium methodological quality were found according to the guidelines used for its evaluation. None of the included studies showed the magnitude of the relationship between pulp necrosis, apical periodontitis and Flare Up. Conclusions: The magnitude of the relationship between pulp necrosis and asymptomatic apical periodontitis with Flare Up could not be established due to the lack of information and data reported by the articles included in this work.spa
