Evaluación de presencia de microcracks en dientes de único conducto utilizando 2 tipos de instrumentación rotatoria. Estudio ex vivo

| dc.description | Aunque la formación de microcracks es un fenómeno frecuente que puede comprometer la integridad dental y aumentar el riesgo de fracturas verticales, no existe consenso en la literatura sobre los factores predisponentes ni cuál sistema es más seguro, determinando evaluar la formación de microcracks durante la instrumentación endodóntica en dientes unirradiculares, comparando los sistemas rotatorios ProTaper Next y Wave One Gold. La hipótesis es que la elección del sistema de instrumentación influye significativamente en la formación de microcracks. Se utilizaron 43 dientes unirradiculares, divididos en tres grupos: Wave One Gold, ProTaper Next y un grupo control. Tras la instrumentación, se evaluó la presencia de microcracks mediante inspección microscópica. Las variables analizadas incluyeron el sistema de instrumentación, la anatomía dental y el operador. Resultados: se demostró que el 62.79% de los dientes no presentaron microcracks. Se encontró una asociación estadísticamente significativa entre el sistema de instrumentación y la formación de microcracks (p = 0.021), siendo el grupo control libre de microcracks. Además, no se observaron asociaciones significativas con la anatomía dental o el operador. La formación de microcracks no es inevitable, pero está influenciada por el sistema de instrumentación utilizado. Los sistemas de instrumentación evaluados demostraron diferencias significativas, con relación a la incidencia de microcracks. Se determinó mayor presencia de estos fenómenos con el sistema Wave One Gold. Es importante que el clínico seleccione sistemas que minimicen el daño dentinario y preserven la integridad estructural del diente durante el tratamiento endodóntico. | |
| dc.description.abstract | Problem: Although microcrack formation is a frequent phenomenon that can compromise dental integrity and increase the risk of vertical fractures, there is no consensus in the literature on the predisposing factors or which system is safer. Objective: To evaluate the formation of microcracks during endodontic instrumentation in uniradicular teeth, comparing the ProTaper Next and Wave One Gold rotary systems. The hypothesis is that the choice of instrumentation system significantly influences microcrack formation. Methods: 43 uniradicular teeth were used, divided into three groups: Wave One Gold, ProTaper Next and a control group. After instrumentation, the presence of microcracks was evaluated by microscopic inspection. The variables analyzed included the instrumentation system, dental anatomy and the operator. Results: It was demonstrated that 62.79% of the teeth did not present microcracks. A statistically significant association was found between the instrumentation system and microcrack formation (p = 0.021), with the control group being free of microcracks. In addition, no significant associations were observed with dental anatomy or operator. Conclusion: Microcrack formation is not inevitable, but is influenced by the instrumentation system used. The instrumentation systems evaluated showed significant differences in relation to the incidence of microcracks. A greater presence of these phenomena was determined with the Wave One Gold system. It is important for the clinician to select systems that minimize dentin damage and preserve the structural integrity of the tooth during endodontic treatment. | |
