Materiales y técnicas de terapia pulpar vital en molares permanentes con caries en adultos: Umbrella Review

dc.description | Introducción: la pulpa dental es un tejido no mineralizado que aporta la vitalidad al diente. En este sentido, cuando la caries dental afecta pulpa, se debe realizar endodoncia convencional. Con la llegada de la odontología mínimamente invasiva se proponen los tratamientos de terapia pulpar vital (VPT) cuyo objetivo es preservar el diente vital retirando la infección causada por caries dental únicamente de la estructura cameral del diente. Objetivo: identificar las alternativas de materiales y técnicas indicadas, para preservar la vitalidad de la pulpa en molares permanentes afectados por lesiones cariosas cavitacionales activas, reportadas en la literatura científica entre 2015-2021. Materiales y métodos: Umbrella review en donde se recopilaron estudios de bases de datos como “PubMed”, “Science Direct”, “Scopus” y “Cochrane Library”. Se realizó la lectura y evaluación de calidad de las revisiones sistemáticas mediante la declaración PRISMA (Preferred Reporting Items for Systematic reviews and Meta-Analyses) y después se agregó la información obtenida a una base de datos en Microsoft Excel. Resultados: Se analizaron 12 revisiones sistemáticas, el 66,7% (8) de la información fue obtenida de PubMed. La técnica con mayor evidencia científica encontrada fue la pulpotomía, seguida del recubrimiento pulpar directo. El agregado trióxido mineral (MTA) reportó como el biomaterial más implementado con tasas de éxito cercanas al 100%, además de ser el más estudiado dentro de los documentos analizados en las revisiones sistemáticas. Conclusiones: El hidróxido de calcio, aunque es el material con mayor investigación, reporta riesgo de fracaso a largo plazo. Por otro lado, el MTA y la Biodentina, utilizados en la pulpotomía y en el recubrimiento pulpar directo en molares permanentes, se evidencian como los materiales de elección con mejores resultados, según la literatura analizada. Se recomienda seguir realizando investigaciones que permitan solidificar estas propuestas para realizar tratamientos mínimamente invasivos exitosos. | spa |
dc.description.abstract | Introduction: the dental pulp is a nonmineralized tissue that provides vitality to the tooth. In this sense, when tooth decay affects pulp, conventional endodontics should be performed. With the advent of minimally invasive dentistry, vital pulp therapy (PTV) treatments are proposed to preserve the vital tooth by removing the infection caused by tooth decay only from the chamber structure of the tooth. Objective: to identify the alternatives of indicated materials and techniques, to preserve the vitality of the pulp in permanent molars affected by active cavitational carious lesions, reported in the scientific literature between 2015-2021. Materials and methods: Umbrella review where database studies such as "PubMed", "Science Direct", "Scopus" and "Cochrane Library" were collected. The systematic reviews were read and evaluated using the PRISMA (Preferred Reporting Items for Systematic reviews and Meta-Analyses) declaration and then the information obtained was added to a database in Microsoft Excel. Results: 12 systematic reviews were analyzed, 66.7% (8) of the information was obtained from PubMed. The technique with the greatest scientific evidence found was pulpotomy, followed by direct pulp coating. Mineral trioxide aggregate (MTA) reported as the most implemented biomaterial with success rates close to 100%, in addition to being the most studied within the documents analyzed in the systematic reviews. Conclusions: Calcium hydroxide, although it is the most researched material, reports risk of long-term failure. On the other hand, MTA and Biodentina, used in pulpotomy and in direct pulp coating in permanent molars, are the materials of choice with the best results, according to the analyzed literature. Further research is recommended to solidify these proposals for successful minimally invasive treatments. | spa |
