Encefalopatía traumática crónica (ETC) en boxeadores: una revisión sistemática de factores pronóstico.

dc.descriptionResumen: La encefalopatía traumática crónica (ETC) es una enfermedad neurodegenerativa que se produce por traumatismos cerebrales repetitivos, donde la proteína tau es la principal responsable, presenta 4 estadios de desarrollo. Existen pocas síntesis de evidencia sobre la ETC que sigan las recomendaciones de reporte y metodológicas de las guías internacionales. Objetivo: Identificar, evaluar y sintetizar los hallazgos de la mejor evidencia empírica para determinar la asociación entre la exposición a traumatismos craneoencefálicos repetitivos sobre la mortalidad por todas las causas, la calidad de vida relacionada con la salud (CVRS) y la atrofia cerebral en boxeadores. Métodos: Esta revisión sistemática se reportó siguiendo los criterios y recomendaciones de la extensión para protocolos de la declaración PRISMA-P. Se buscó en las bases de datos MEDLINE (vía PubMed), SCOPUS, Embase (Elsevier), EBSCO (vía SPORTDiscus), repositorios de literatura gris como Google académico y se cribó en repositorios de registros de estudios como Clinical Trials. La revisión se registró en Open Science Framework (OSF) Doi: https://doi.org/10.17605/OSF.IO/35DAQ Se hizo una evaluación del riesgo de sesgo. Varias etapas de la revisión fueron desarrolladas mínimo por dos revisores cegados y de forma independiente. Resultados: Examinamos 654 citas únicas para incluir 9 estudios (1065 participantes). Las poblaciones del estudio fueron en su mayoría boxeadores profesionales y aficionados. Para el desenlace de mortalidad se reportó 1 estudio con datos de 10 boxeadores, pero únicamente 1 diagnosticado con ETC. En el desenlace de atrofia cerebral se detectaron 4 estudios con reducción del volumen del tálamo, del hipocampo, hipotálamo, amígdala, núcleo caudado, putamen, cavum septum pellucidum, enfermedad de la sustancia blanca periventricular y enfermedad de la sustancia blanca subcortical. Por último en el desenlace de calidad de vida relacionada con la salud, en 7 (78%) estudios, la cohorte expuesta dio como resultado un desempeño inferior en comparación a la cohorte no expuesta, o no se reportaron cambios significativos entre las dos cohortes en las pruebas indirectas. Conclusión: La evidencia actual no es suficiente, debido a que ningún estudio presentó el diagnóstico del factor pronóstico de interés, es decir, la ETC y su presencia en boxeadores, porque, aunque uno de los estudios incluidos si diagnosticó la enfermedad, no hubo un seguimiento del factor pronóstico en vida de los deportistas que permitiera ver el curso clínico neurodegenerativo. Por ello, recomienda realizar más estudios observacionales a futuro que midan adecuadamente los desenlaces abordados en esta revisión.spa
dc.description.abstractAbstract Chronic traumatic encephalopathy (CTE) is a neurodegenerative disease caused by repetitive brain trauma, where the tau protein is the main responsible, with 4 stages of development. There are few evidence syntheses on CTE that follow the reporting and methodological recommendations of international guidelines. Objective: To identify, evaluate and synthesize the findings of the best empirical evidence to determine the association between exposure to repetitive traumatic brain injury on all-cause mortality, health-related quality of life (HRQoL) and brain atrophy in boxers. Methods: This systematic review was reported following the criteria and recommendations of the protocol extension of the PRISMA-P statement. We searched MEDLINE (via PubMed), SCOPUS, Embase (Elsevier), EBSCO (via SPORTDiscus), gray literature repositories such as Google Scholar, and screened study registry repositories such as Clinical Trials. The review was registered in the Open Science Framework (OSF). A risk of bias assessment was performed. Several stages of the review were performed by at least two blinded and independent reviewers. Results: We examined 654 unique citations to include 9 studies (1065 participants). Study populations were mostly professional and amateur boxers. For the mortality outcome, 1 study was reported with data from 10 boxers, but only 1 diagnosed with CTE. For the outcome of brain atrophy, 4 studies were detected with reduced volume of the thalamus, hippocampus, hypothalamus, amygdala, caudate nucleus, putamen, cavum septum pellucidum, periventricular white matter disease, and subcortical white matter disease. Finally on the health- related quality of life outcome, in 7 (78%) studies, the exposed cohort resulted in inferior performance compared to the unexposed cohort, or no significant changes were reported between the two cohorts on the indirect tests. Conclusion: The current evidence is not sufficient, because no study presented the diagnosis of the prognostic factor of interest, that is, CTE and its presence in boxers, because, although one of the included studies did diagnose the disease, there was no follow-up of the prognostic factor during the lifetime of the athletes that would allow us to see the neurodegenerative clinical course. Therefore, it is recommended to perform more observational studies in the future that adequately measure the outcomes addressed in this review.spa
