Reproducibilidad de la alineación dental en medidas digitales y manuales en modelos iniciales de pacientes de ortodoncia de la Universidad Santo Tomás de Bucaramanga

dc.descriptionAntecedentes: La introducción de modelos digitales ofrece al ortodoncista una alternativa a los modelos de estudio de yeso que se utilizan habitualmente. Los modelos de estudio de yeso son un componente estándar de los registros de ortodoncia y son fundamentales para el diagnóstico y la planificación del tratamiento. Objetivos: Establecer la reproducibilidad de las medidas digitales y manuales de alineación dental en modelos iniciales de pacientes de ortodoncia realizados en las clínicas de la Universidad Santo Tomás desde febrero de 2018 a junio de 2019. Materiales y métodos: Se realizó un estudio de evaluación de tecnología diagnóstica debido a que se evaluó la confiabilidad en las medidas digitales y manuales en modelos de pacientes nuevos de ortodoncia de las clínicas de la Universidad Santo Tomás de I semestre de 2018 a I semestre de 2019. Se obtuvo un universo de ochenta modelos y se realizó un muestreo probabilístico aleatorio simple para obtener la muestra, se digitalizaron los modelos de abril de 2019 a noviembre del mismo año con el Escáner Ineos X5(escáner CAD/CAM de Sirona). Posteriormente se calibró a cada uno de los investigadores y una vez obtenidos los modelos en yeso y las imágenes digitales de estos, el investigador que obtuvo el mejor resultado en la calibración inter e intra examinador, realizó la medición de la alineación dental. Las medidas manuales se llevaron a cabo con un calibrador digital, mientras que las digitales se tomaron con el software Nemocast., una vez concluida la fase de recolección de datos, la base se exportó al programa estadístico Stata I/C versión 14 para su análisis. La reproducibilidad de las mediciones manuales y digitales se evaluó mediante el CCI y se estableció el intervalo de confianza del 95%, donde valores de 0,81 a 1,00 se consideran “casi perfectos”. Los límites de acuerdo de Bland y Altman se evaluaron según la distribución gráfica de la diferencia de los promedios, un valor cercano a cero indicó un mayor acuerdo entre los datos manuales y digitales. Un valor de p<0,05 fue considerado como estadísticamente significativo. Resultados: Se evaluaron 80 modelos de estudio, que correspondían a 43 mujeres (53,8%) y 37 hombres (46,2%). Los valores del Coeficiente de Correlación Intraclase oscilaron entre 0,643 a 0,874. La medida que presentó el más alto Coeficiente de Correlación Intraclase fuero la distancia entre el canino y el lateral derecho superior [ICC: 0,874 IC 95%: 0,786 - 0,924]. De otro lado, que obtuvo el valor más bajo fue la distancia entre el lateral y el central derecho inferior [ICC: 0,643 IC 95%: 0,344 - 0,813] al comparar con las demás medidas tomadas. Cabe señalar que más de la mitad de las mediciones obtuvieron valores superiores a 0,81 lo que se considera una reproducibilidad “casi perfecta” según la escala sugerida por Landis y Koch. Conclusiones: Al evaluar las mediciones realizadas en modelos digitales y manuales se encontró una alta reproducibilidad entre ellas, ya que los Coeficientes de Correlación Intraclase indicaron una concordancia “casi perfecta “entre ambos grupos.spa
dc.description.abstractBackground: The introduction of digital models offers the orthodontist an alternative to the commonly used gypsum study models. Gypsum study models are a standard component of orthodontic records and are central to diagnosis and treatment planning. Objectives: To establish the reproducibility of digital and manual measurements of dental alignment in initial models of orthodontic patients performed at the Santo Tomás University clinics from February 2018 to June 2019. Materials and methods: A diagnostic technology evaluation study was carried out because the reliability of digital and manual measurements was evaluated in new orthodontic patient models from the Santo Tomás University clinics from the first semester of 2018 to the first semester of 2019. A universe of eighty models was obtained and a simple random probability sampling was carried out to obtain the sample, the models were digitized from April 2019 to November of the same year with the Ineos X5 Scanner (Sirona CAD / CAM scanner). Subsequently, each of the researchers was calibrated and once the plaster models and their digital images were obtained, the researcher who obtained the best result in the inter and intra-examiner calibration performed the dental alignment measurement. Manual measurements were carried out with a digital caliper, while digital measurements were taken with Nemocast software. Once the data collection phase was completed, the base was exported to the Stata I / C version 14 statistical program for analysis. The reproducibility of manual and digital measurements was evaluated using the CCI and the 95% confidence interval was established, where values from 0.81 to 1.00 are considered “almost perfect”. The Bland and Altman limits of agreement were evaluated according to the graphic distribution of the difference of the averages, a value close to zero indicated a greater agreement between manual and digital data. A value of p <0.05 was considered statistically significant. Results: 80 study models were evaluated, corresponding to 43 women (53.8%) and 37 men (46.2%). Intraclass Correlation Coefficient values ranged from 0.643 to 0.874. The measure that presented the highest Intraclass Correlation Coefficient was the distance between the canine and the superior right lateral [CHF: 0.874 95% CI: 0.786 - 0.924]. On the other hand, the lowest value was the distance between the lateral and the lower right central [CHF: 0.643 95% CI: 0.344 - 0.813] when compared with the other measurements taken. It should be noted that more than half of the measurements obtained values greater than 0.81, which is considered a “near perfect” reproducibility according to the scale suggested by Landis and Koch. Conclusions: When evaluating the measurements made in digital and manual models, a high reproducibility was found between them, since the Intraclass Correlation Coefficients indicated a "near perfect" agreement between both groups.Key words: Reproducibility of results, Model, Measurements.spa
