dc.description | Introducción La diabetes mellitus (DM), es una enfermedad crónica que abarca un grupo de desórdenes heterogéneos con la característica común de la tolerancia a la glucosa y un metabolismo de carbohidratos y lípidos alterados. Diferentes estudios han evaluado la relación bidireccional entre la DM y la enfermedad periodontal, ya sea por mecanismos directos o indirectos. Dentro de los dispositivos portátiles disponibles para evaluar el control de hemoglobina glicosilada (HbA1c) se encuentra el HbA1c, permitiendo una toma de muestra rápida que se puede emplear durante la consulta odontológica. Objetivo Establecer la equivalencia de la medición de HbA1c en sangre crevicular gingival de personas con diabetes mellitus tipo 2, con la medición de HbA1c en sangre venosa en pacientes con diabetes mellitus tipo 2 que acuden a las clínicas odontológicas de la Universidad Santo Tomás campus Floridablanca. Materiales y métodos Se realizó un estudio para evaluación de pruebas diagnósticas. Se realizó un muestreo de tipo corte trasversal, en donde se evaluaron 20 personas adultas con diagnóstico de diabetes mellitus tipo 2 que asistieron a la clínica odontológica de la Universidad Santo Tomas de Floridablanca. Se evaluó el porcentaje de hemoglobina glicosilada en sangre venosa y sangre crevicular gingival en diente sano y diente con bolsa periodontal. Para variables cuantitativas se realizó el cálculo de medias, medianas y medidas de dispersión (DE, RI) de acuerdo con el análisis de normalidad realizado con la prueba de Shapiro Wilk. Se compararon los resultados de las dos pruebas por medio de la prueba de Bland y Altman. Se utilizó el coeficiente de correlación intraclase (CCI) para determinar la reproducibilidad de la medición de HbA1c en sangre crevicular y venosa (en su forma continua). Resultados Se encontró que los coeficientes de correlación intraclase y los intervalos de confianza del 95% de los resultados de la toma de HbA1c entre los tres métodos usados obtuvo una reproducibilidad casi perfecta, pues los CCI oscilaron entre 0.91 y 0.96. También se observa que el promedio de la diferencia entre las mediciones de HbA1c con cada uno de los métodos no alcanzan el medio punto porcentual. Los límites de acuerdo de 95% de Bland y Altman fueron aproximadamente +- 1.5% para todos los métodos. Conclusión Los niveles de HbA1C en sangre crevicular comparado con sangre venosa muestran concordancia, lo cual significa que la medición HbA1C mediante el dispositivo portátil A1CNow puede utilizarse como método en control para paciente diabético durante la consulta odontológica. Se recomienda considerar el presente estudio como prueba piloto para próximas investigaciones. | spa |
dc.description.abstract | Introduction Diabetes mellitus (DM) is a chronic disease that encompasses a group of heterogeneous disorders with the common characteristic of glucose tolerance and a metabolism of altered carbohydrates and lipids. Different studies have evaluated the bidirectional relationship between DM and periodontal disease, either by direct or indirect mechanisms. HbA1c is available, allowing a rapid sample taking that can be used during the dental visit. Objective To establish the equivalence of the measurement of HbA1c in gingival crevicular blood of people with diabetes mellitus type 2, with the measurement of HbA1c in venous blood in patients with type 2 diabetes mellitus who attend the dental clinics of the Santo Tomás University campus Floridablanca. Materials and methods A study was conducted to evaluate diagnostic tests. A cross-section type sampling was carried out, in which 20 adults diagnosed with type 2 diabetes mellitus who attended the dental clinic of the Santo Tomas de Floridablanca University were evaluated. The percentage of glycosylated hemoglobin in venous blood and gingival crevicular blood in healthy tooth and tooth with periodontal pocket was evaluated. For quantitative variables, the calculation of means, medians and dispersion measures (SD, IR) was performed according to the normality analysis performed with the Shapiro Wilk test. The results of the two tests were compared by means of the Bland and Altman test. The intraclass correlation coefficient (ICC) was used to determine the reproducibility of the HbA1c measurement in crevicular and venous blood (in its continuous form). Results It was found that the intraclass correlation coefficients and the 95% confidence intervals of the HbA1c intake results between the three methods used obtained an almost perfect reproducibility, since the CCI ranged between 0.91 and 0.96. It is also observed that the average difference between the HbA1c measurements with each of the methods does not reach half a percentage point. The 95% agreement limits of Bland and Altman were approximately + - 1.5% for all methods. Conclusion HbA1C levels in crevicular blood compared to venous blood are concordant, which means that the HbA1C measurement using the portable device A1CNow can be used as a control method for diabetic patients during the dental visit. It is recommended to consider the present study as a pilot test for future investigations. | spa |
