Evaluación mecánica de la unión diente-implante con prótesis parcial fija: análisis por el método de elementos finitos

dc.descriptionLa rehabilitación oral, se encarga de plantear opciones protésicas, con el fin de lograr un tratamiento con diferentes materiales restauradores, en pacientes parcialmente dentados los tratamientos usados son; prótesis parciales removibles, prótesis parciales fijas implanto soportadas, prótesis parciales fijas dento soportadas y prótesis parciales fijas dento implanto soportadas. El presente estudio evalúo la mecánica de la unión diente-implante en prótesis parcial fija con implante cono Morse. Con la incursión de los análisis del elemento finito en los estudios odontológicos, se han logrado hacer pruebas muy cercanas a la realidad y se pueden hacer estimaciones valiosas en prostodoncia e implantología. Objetivo: Analizar los desplazamientos totales de un modelo virtual de prótesis parcial fija implanto-dento soportada, distribución de esfuerzos principales en la superficie ósea periodontal y peri-implantar, distribución y esfuerzos principales en pilar protésico e implante en el modelo de prótesis parcial fija implanto-dento soportada, mediante el método de los elementos finitos. Materiales y métodos: Se realizó un estudio experimental de simulación, por el Método de los Elementos Finitos (MEF), con una muestra virtual, no probabilística. En el modelo a diseñar en Solidworks 2015, el primer premolar diente 24 relacionado a diente pilar y tejidos de soporte, el segundo premolar como póntico y el primer molar es un implante Cone Morse. El Grupo 1-experimental: Prótesis parcial fija implant-dento soportada (24-25-26) cementada sobre diente pilar (24), póntico (25) y pilar-implante CM (26). Grupo 2-control: Prótesis parcial fija dento soportada (24-25-26) cementada sobre diente pilar (24), póntico (25) y diente pilar (26). Grupo 3-control: Prótesis parcial fija Implanto soportada (24-25-26) cementada sobre pilar-implante CM (24) y pilar-implante CM (26). Resultados: Diente-implante: muestra un desplazamiento total del modelo implante-diente, cuyo valor máximo de 0.010038 [mm] es obtenido en la cerámica da la corona del diente pilar 24, esto indica que la menor rigidez del diente (menor módulo de elasticidad) produce un mayor desplazamiento. Los desplazamientos máximos totales encontrados en la cerámica del diente pilar 24 y póntico 25, son transmitido a la región disto-vestibular del preparo coronal del diente 24, siendo más del doble que los hallados en la región del pilar, con valores máximos de 0.0096 [mm] y 0.0042 [mm] respectivamente. Diente-diente: Al revisar los desplazamientos totales en el modelo, se observa que en este caso se alcanza una distribución uniforme, encontrando la zona más desplazada en el póntico 25, con valor de desplazamiento máximo de 0.0105 [mm]. Implante-implante: Los desplazamientos totales máximos del modelo Implante-Implante se encuentran en la corona protésica pilar de 24, con un valor 0.0104 [mm]. Conclusiones: En prótesis implanto-dento soportada existe mayor desplazamiento del diente pilar en relación a la región del pilar-implante. Existe mayor desplazamiento en el diente pilar de una prótesis implanto-dento soportada en relación a la prótesis dento soportada. La superficie ósea periodontal presenta un funcionamiento mecánico similar cuando son rehabilitados con prótesis implanto-dento soportada o prótesis dentosoportadas. El pilar protésico presenta comportamiento mecánico similar cuando es utilizado sobre prótesis implanto dento soportadas o implantosoportada.spa
dc.description.abstractOral rehabilitation is in charge of proposing prosthetic options, in order to achieve a treatment with different restorative materials. In partially dentate patients, the treatments used are; removable partial dentures, supported implant fixed partial prostheses, supported dento fixed partial prostheses, and implant supported fixed partial dentures. The present study evaluated the mechanics of the tooth-implant junction in a fixed partial prosthesis with a Morse cone implant. With the incursion of finite element analysis in dental studies, tests very close to reality have been made and valuable estimates can be made in prosthodontics and implantology. Objective: To analyze the total displacements of a virtual model of supported implant-dento fixed partial prosthesis, distribution of main stresses in the periodontal and peri-implant bone surface, distribution and main stresses in the prosthetic abutment and implant in the model of fixed partial implant prosthesis. -dento supported. using the finite element method. Materials and methods: An experimental simulation study was carried out using the Finite Element Method (MEF), with a virtual, non-probabilistic sample. Results: Tooth-implant: shows a total displacement of the implant-tooth model, whose maximum value of 0.010038 [mm] is obtained in the ceramic of the crown of the abutment tooth 24, this indicates that the lower stiffness of the tooth (lower modulus of elasticity ) produces a greater displacement. The total maximum displacements found in the ceramic of the abutment tooth 24 and pontic 25, are transmitted to the disto-vestibular region of the coronal preparation of tooth 24, being more than double that found in the abutment region, with maximum values of 0.0096 [ mm] and 0.0042 [mm] respectively. Tooth-tooth: When reviewing the total displacements in the model, it is observed that in this case a uniform distribution is reached, finding the most displaced area in the pontic 25, with a maximum displacement value of 0.0105 [mm]. Implant-implant: The maximum total displacements of the Implant-Implant model are found in the abutment prosthetic crown of 24, with a value of 0.0104 [mm]. Conclusions: In supported implant-dento prostheses there is a greater displacement of the abutment tooth in relation to the abutment-implant region. There is greater displacement in the abutment tooth of a supported implant-dento prosthesis in relation to the supported dento prosthesis. The periodontal bone surface presents a similar mechanical function when they are rehabilitated with supported implant-dento prostheses or dento-supported prostheses. The prosthetic abutment exhibits similar mechanical behavior when used on implant-supported or implant-supported prostheses.spa
