Abstract Introduction: The choice of intracanal medication depends on having an accurate diagnosis of the condition being treated, as well as a thorough understanding of the type of organisms likely involved. Calcium hydroxide, which has low water solubility and slowly releases calcium (Ca²⁺) and hydroxyl (OH⁻) ions, remains the most predictable antibacterial agent. Its primary function is to control infection in the root canal systems of necrotic teeth and promote periapical healing. Another recently introduced medication is Bio C Temp®, which has limited scientific evidence. Objective: To compare the calcium ion penetration in dentinal tubules of Bio C Temp® with Calcifar® calcium hydroxide after ultrasonic activation using confocal laser scanning microscopy (CLSM). Method: Thirty human single-rooted teeth were selected and randomly divided into two groups of 15 teeth: one treated with Calcifar® and the other with Bio C Temp®. The samples were processed, and the penetration of the intracanal medications into the dentinal tubules was measured in microns using CLSM. Statistical analysis included an independent samples t-test. Results: Both intracanal medications showed penetration into the dentinal tubules, with an average penetration of 1329.584 μm for Calcifar® and 959.775 μm for Bio C Temp®. However, no statistically significant difference was found between the two intracanal medications (p-value = 0.076). Conclusions: Calcifar® and Bio C Temp® provide appropriate penetration into the dentinal tubules of the apical third. Although Calcifar® exhibited greater penetration, this difference was not significant, suggesting that both intracanal medications are viable options for endodontic treatment.
