Introducción: El uso de aparatología ortopédica maxilar, aunque es fundamental para corregir maloclusiones y alteraciones craneofaciales, puede llegar a generar repercusiones psicológicas y sociales afectando el bienestar emocional y la calidad de vida por dificultades de adaptación, cambios estéticos e intimidación escolar. Objetivo: Determinar el nivel de malestar psicológico y su impacto en la calidad de vida de niños y adolescentes tratados con aparatología ortopédica maxilar en las clínicas odontológicas de la Universidad Santo Tomás en Piedecuesta Santander, durante el año 2025 y el primer semestre de 2026. Método: Se realizó un estudio observacional de corte transversal analítico en una población de 113 pacientes entre 6 y 13 años que recibían tratamiento ortopédico maxilar. La información se recolectó mediante cuestionarios e instrumentos validados orientados a evaluar malestar psicológico, intimidación asociada al uso del aparato y calidad de vida relacionada con la salud oral. Se incluyeron variables sociodemográficas, tipo de aparatología y percepción del bienestar general. Resultados: El 13,04% de los hombres reportaron haber experimentado burlas o molestias relacionadas con el uso de aparatos, sin diferencias significativas entre ambos grupos (p [LL1.1]= 0,92). El 75 % de los pacientes (n=18) presentó mejora significativa en la calidad de vida tras el tratamiento (p=0,01). Los puntajes medios post-tratamiento en bienestar emocional (M=8,2, SD=1,0) e imagen corporal (M=7,5, SD=1,3) fueron mayores que al inicio (p<0,05). Un 20 % de la muestra no mostró cambios notables. Estos hallazgos indican un efecto positivo del tratamiento con aparatología. Conclusiones: El tratamiento de ortopedia maxilar trasciende la corrección funcional y estética, ya que también influye en el bienestar psicológico y social de niños y adolescentes. Se resalta la necesidad de incorporar el abordaje emocional dentro del seguimiento clínico ortopédico, con el fin de favorecer la adaptación al tratamiento y mejorar la adherencia a este.

Abstract

Background: The use of maxillary orthopedic appliances, although essential for correcting malocclusions and craniofacial abnormalities, may lead to psychological and social repercussions that negatively affect emotional well-being and quality of life due to adaptation difficulties, esthetic changes, and school bullying. Objective: To determine the level of psychological distress and its impact on the quality of life of children and adolescents treated with maxillary orthopedic appliances at the dental clinics of Universidad Santo Tomás in Piedecuesta, Santander, during 2025 and the first semester of 2026. Methods: An analytical cross-sectional observational study was conducted in a population of 113 patients aged 6 to 13 years undergoing maxillary orthopedic treatment. Data were collected using validated questionnaires and instruments designed to assess psychological distress, appliance-related bullying, and oral health-related quality of life. Sociodemographic variables, type of orthopedic appliance, and perceived overall well-being were also included. Results: A total of 13.04% of male participants reported experiencing teasing or bullying related to wearing orthopedic appliances, with no statistically significant differences between groups (p = 0.92). In addition, 75% of the patients (n = 18) showed a significant improvement in overall quality of life after treatment (p = 0.01). Mean post-treatment scores for emotional well-being (M = 8.2, SD = 1.0) and body image (M = 7.5, SD = 1.3) were significantly higher than baseline values (p < 0.05). Twenty percent of the sample showed no relevant changes. Overall, these findings indicate a positive effect of treatment with maxillary orthopedic appliances. Conclusions: Maxillary orthopedic treatment extends beyond functional and esthetic correction, as it also has a positive influence on the psychological and social well-being of children and adolescents. These findings highlight the importance of incorporating emotional assessment and support into routine orthopedic follow-up to facilitate treatment adaptation and improve treatment adherence.
