This research evaluated the effect of an 8-week supervised Pilates Reformer program (16 sessions, 50–60 min, twice weekly) on adult patients diagnosed with Fibromyalgia in the city of Tunja, Boyacá.The study employed a quasi-experimental design with pre- and post-intervention measurements and convenience sampling; the instruments used included the VAS for pain, FIQR for disease impact, FSS for fatigue, and the Sit-and-Reach test for flexibility. The study sample presented anthropometric characteristics typical of a predominantly female population with a BMI in the overweight range.Following the intervention, significant improvements were observed in the four main variables analyzed: reduction in perceived pain (VAS) (p = 0.033), decrease in the global impact of the disease (FIQR) (p = 0.028), lower fatigue severity (FSS) (p = 0.041), and increased flexibility (Sit-and-Reach) (p = 0.022). The effects, generally moderate, are consistent with plausible mechanisms such as motor control retraining, load progression mediated by the reformer springs, and psychosocial benefits associated with guided practice. This study provides local evidence regarding the Reformer modality and its feasibility in a rehabilitation context, although its findings should be interpreted with caution given the methodological limitations (small sample size, convenience sampling, lack of randomization, and lack of long-term follow-up).
