The general objective is to design a structured business plan for the creation of an external auditing consultancy firm specifically tailored to satisfy the technical, scientific, and administrative compliance needs of these institutions. Methodological DesignThe study is descriptive with a mixed approach (qualitative and quantitative). It was sequentially structured into five phases: market study, organizational/legal design, economic-financial study, technical study, and the development of the value proposition. For primary data collection, a 30-question Likert-type survey (validated by experts) was administered to a convenience sample of 68 dental service providers in the Metropolitan Area of Bucaramanga. Results and Business ProposalCorporate Structure: The incorporation of the company was established under the name Smile Audit Group SAS (SAG), categorized under the commercial activity of management consultancy (ISIC/CIIU code 7020). Location: Using a qualitative point-scoring method, the optimal location for the commercial office was determined to be the Cañaveral zone in Floridablanca, Santander, due to advantages in accessibility, lease costs, and proximity to clients. Economic-Financial Study: The total investment required for the startup amounts to $309,019,462 COP. This is divided into fixed investment ($50,434,000 COP), deferred investment ($17,090,000 COP), and working capital ($241,495,462 COP). The financing strategy consists of 32.36% from partners' equity contributions and 67.64% through a 72-month bank credit line. Value Proposition: A user manual focused on auditing dental offices was developed, grounded in the PDCA cycle (Plan, Do, Check, Act) and adapted to the different levels of complexity and biosecurity required by Colombian regulations. Conclusions and ViabilityThe financial study demonstrated high feasibility and long-term business sustainability, yielding an Internal Rate of Return (IRR) of 53.97% and a Net Present Value (NPV) that significantly exceeds 400 million COP. This research project arises from the persistent gaps in the effective guarantee of the fundamental right to health, which is directly related to the well-being and comprehensive development of the population. The right to health, a pillar of human dignity, must be guaranteed equitably and promptly for all citizens, without discrimination. This principle is supported by the Political Constitution of Colombia and the Statutory Health Law (Law 1751 of 2015), which recognize health as an autonomous fundamental right. However, reality reveals significant deficiencies in the access, quality, and timeliness of services, making the search for innovative solutions essential. This research aligns with the 2022-2026 National Development Plan, which aims to strengthen the health system and reduce inequalities, especially among vulnerable populations and historically underserved regions. It also contributes to the achievement of the Sustainable Development Goals, particularly SDG 3 (Good Health and Well-being) and SDG 10 (Reduced Inequalities). The study will address access to healthcare as a multidimensional challenge, analyzing factors such as infrastructure, human resources, geographic and economic accessibility, service quality and cultural acceptability, as well as the social determinants of health. In response, a humanized care model has been developed, positioning women, their newborns, and families as central figures in the birthing process. This qualitative, documentary-based study aims to describe the practices associated with humanized childbirth and assess its benefits for women's comprehensive health within the South American context. A systematic literature review was conducted across seven reliable databases: ScienceDirect, Scopus, Scielo, PubMed, Dialnet, ProQuest and Google Scholar. A total of 26 articles published between 2015 and 2025 were selected based on predefined inclusion criteria. Thematic analysis revealed two main categories—(1) practices related to humanized childbirth and (2) health benefits for women—and one emerging category: (3) women’s perceptions and experiences during labor and delivery. The findings indicate limited institutional adoption of humanized birth practices, hindered by procedural, structural, and staffing barriers. Therefore, this proposal seeks to evaluate the causes of non-reporting, as a fundamental aspect to analyze, detect and record the causes that influence adherence to the reporting of adverse events generated by patients. This, through a case study, in a first-level institution in Boyacá located in the municipality of Campohermoso, with the application of questionnaires to workers who are immersed in the reporting process. With the results, the aim is to promote a culture of patient safety where the notification of adverse events and their adequate plan for improvement and analysis are promoted to renew the care directed to the patient. Therefore, this research aims to analyze the perception of nursing staff on the management and institutional commitment of the Clínica de los Andes to patient safety, through a descriptive research, with a quantitative approach, applied to 80 nursing assistants among nursing professionals and nursing assistants of the Clínica de los Andes, by applying the survey ‘Hospital Survey on Patient Safety Culture’ (HSOPSC) of the Agency of Healthcare Research and Quality (AHRQ) Spanish version, which evaluates 12 dimensions for patient safety culture, through 51 closed questions formulated in negative and positive and to assess each item a Likert scale of five degrees is used according to each section. The expected results will allow us to explore the factors that affect patient safety, such as the failure in communication between health teams and the fear of punitive actions when reporting adverse events that occur during the health care process. Furthermore, the implementation of the instrument will show both strengths and opportunities for improvement in strategies aimed at patient safety, either individually or generally, implemented by the health institution. Objective: To analyze the use of remote telemetry in patients with implantable cardiac devices by means of a systematic review of the literature in order to provide an overview of its implementation in the department of Boyacá. Methodology: A descriptive and qualitative systematic literature review was carried out in Scielo, PubMed and Google Scholar databases, using as keywords: Implantable cardiac devices and remote monitoring, finding 66 articles and applying exclusion criteria, 14 articles were selected for the research. The purpose of this research is to identify the psychosocial factors that influence the humanization of mental health services in a military health facility in Colombia. The above, through a quantitative and descriptive approach, a questionnaire will be applied to a population universe of 20 health professionals with assistance functions who work in the BASAN hospital rehabilitation center. There are three phases for the development of the research: documentary review, data collection and analysis of results. The research proposal is relevant given the high demand for mental health services in Colombia and the need to improve their quality. For its approach, the regulations and guidelines established by the World Health Organization, the Pan American Health Organization and the Ministry of Health and Social Protection of Colombia are considered. The results that will be obtained in the research stand out for health professionals, institutions and the population in general. On the other hand, safety culture is understood as a set of values, beliefs and attitudes shared by a health organization, which will ensure the protection and well-being of patients. In mental health, safety takes on special relevance due to the vulnerability of patients, who may face additional risks such as aggressive behavior, self injury and even suicide attempts. The aim of this proposal is to evaluate the culture of patient safety in mental health care at the Centro de Rehabilitación Integral de Boyacá in order to strengthen care and reduce risks for patients. A descriptive cross-sectional study with a quantitative approach will be carried out with the application of the instrument “Hospital Survey on Patient Safety Culture” of the Agency of Healthcare Research and Quality (AHRQ) in a sample of 92 workers. In this way, the effectiveness of treatments is promoted and patient satisfaction is improved. Aim: To evaluate the quality of health services in the area of specialized dentistry in an IPS in Arauca in the first half of 2024. Method: Observational, descriptive and cross-sectional study. The information was collected during the first half of 2024, taking as the study population the users treated in the specialized dentistry area of an IPS in Arauca. The SERVQUAL questionnaire was used as a measurement instrument, which investigates the quality of health care through 22 questions and in 5 domains. Results: In a total of 171 patients, the overall results of the IPS showed a general average of 5.20, which suggests a favorable assessment. Similarly, all dimensions showed averages greater than 5, with the Empathy dimension being the highest (5.35) and the Responsiveness dimension the lowest (5.06). Conclusions: Patients in the IPS dentistry area positively perceive the quality of the service, although there are some variables scored below the acceptable average, such as Tangibility and Responsiveness, which shows difficulties with the lack of effectiveness on the part of IPS employees when having specialized dentistry services available.Tipo de ítem: Ítem , Análisis de las glosas por pertinencia en el año 2022 de la E.S.E Hospital Regional de García Rovira(Universidad Santo Tomás, 2025-01-21) Portilla Portilla, Alvaro; Castellanos Caballero, Yuli Mabel; León Gaines, Andrés RicardoProblem: Over the past four years, the E.S.E. HR García Rovira hospital has significantly increased its services, resulting in a proportional rise in pertinent glosses. The lack of an efficient management system for these glosses has caused substantial economic losses. Objective: This study examined the frequency and financial impact of relevance glosses in 2022 to establish a historical baseline and identify improvement strategies for managing these discrepancies. Method: A quantitative, descriptive, and retrospective study was conducted, analyzing glosses reported in 2022. Data were gathered from the hospital's audit files, focusing on frequency, acceptance, and economic losses. Results: In 2022, 12,109 glosses were reported; 159 (1.33%) were due to relevance, causing a financial loss of $7,075,539, accounting for 34.1% of total losses. Discussion: Although relevance glosses were few, their financial impact was significant. The research employs a qualitative methodology, based on documentary analysis of articles, scientific journals, and essays, complemented by the study of specific medical malpractice cases. Interviews with a surgeon and a criminal lawyer are conducted to obtain specialized perspectives. The analysis focuses on the application of Law 23 of 1981 and Law 599 of 2000, which regulate medical ethics and criminal liability respectively. The results suggest that the determination of criminal liability in these cases depends on factors such as adherence to medical protocols, risk assessment, and the existence of informed consent. The objective of this work is to conduct a review of the Tracer Patient methodology at the FOSUNAB Foundation, applied from 2021 to 2023, addressing the problem: Is the FOSUNAB Foundation's Tracer Patient quality management tool effectively fulfilling its role in managing continuous improvement of accreditation requirements, patient safety, and the delivery of services with high-quality standards? To answer this, a descriptive-explanatory study was conducted on the functioning and effectiveness of the Tracer Patient methodology at FOSUNAB, through data collection with the process leadership team and a document review from 2021, 2022, and 2023. It was identified that 49 applications were made, and the compliance percentages did not show a sustained development and strengthening curve, with global results of 87%, 83%, and 87%, respectively. The most evaluated services were: Surgery, Hospitalization, Heart Institute, Clinical Laboratory, Pulmonologists, Health Professionals, and ICU, generating 92 opportunities for improvement. It was concluded that the tracer tool is effectively fulfilling its role in managing continuous improvement of accreditation requirements, patient safety, and the delivery of services with high-quality standards. These discrepancies, which are objections to the amounts billed to insurers, have significantly impacted the hospital's revenues, jeopardizing its economic sustainability. The deductions applied by insurers arise from billing errors, regulatory interpretations, and discrepancies in the application of contractual tariffs. To address this problem, a thorough analysis of the billing processes, documentation, and staff training was conducted. The results revealed that the main deficiencies stem from a lack of training on current regulations and errors in service coding, leading to inconsistencies in issued invoices. Additionally, it was found that the billing process lacks automation, increasing the likelihood of human errors. In response to these findings, the study proposes several strategies to reduce the incidence of billing discrepancies. It suggests automating the billing process to decrease coding errors and facilitate the review of amounts billed. It also recommends strengthening staff training on regulations and updated information systems, as well as implementing quality controls at each stage of the billing process. This is due to the interaction between system-related factors and human actions, which could lead to severe consequences, including death. The objective of this project is to propose strategies to strengthen the patient safety program, aiming to contribute to providing safe care to all users during their stay in the emergency and/or observation service at ESE HSR in Concepción, Santander. Method: A qualitative descriptive study was conducted at a first-level healthcare institution. A sample of 118 users was taken from the 170 who attended the emergency and outpatient services during July 2023. A survey on patient safety perception was used for data collection, and the findings were analyzed to interpret the diagnosis provided and to create valid tools to mitigate the occurrence of adverse events in the institution. Results: Weaknesses were found in the application of aseptic techniques, waiting times, and inadequate communication between patients and healthcare staff, which affected the perception of service quality. According to the World Health Organization (WHO, 2024), diagnostic errors account for 16% of preventable harm in healthcare. Therefore, the goal is to strengthen both individual and collective actions among the various actors involved in service delivery, including patients, their families, healthcare professionals, and institutions. To enhance accurate and timely diagnosis, it is necessary to address organizational-level deficiencies that contribute to breakdowns in communication and the professional-patient relationship, work overload, and shortcomings in the availability of diagnostic supplies and tools. The objective is to prevent delayed, incorrect, or absent diagnoses, which could lead to disease progression, disability, death, or increased costs for the healthcare system. The central Problem lies in the uncertainty generated by the reform, which raises questions about its effect on user perception and service quality. The research method employed a qualitative approach with an explanatory scope and a bibliographic design, analyzing data from the National Health Superintendence and comparing PQRDs from 2023 with those from 2022 to determine any changes. The Results revealed an increase in PQRDs in 2023, mainly associated with barriers to access to health services for users. This growing trend in PQRDs highlights users' concerns about access and quality of services within the context of the reform. The comparison with similar reforms in Chile and Peru underscores common challenges in the region. The Conclusion reflects that the reform and the proposed new changes in healthcare must address these concerns to guarantee an equitable and efficient health system, recommending transparent and participatory management to strengthen user confidence.
